What Romanians die of

Of every thousand deaths in Romania, 545 come from diseases of the heart and the blood vessels. Count the same deaths as years of life lost before 75 and 297 remain. The canvas rearranges itself, and where the red used to be there rise the liver, the road and the first days of life.

The canvas

Each dot is a thousandth of a year of deaths: 242 people.

The thousand dots are every death recorded in Romania in 2023 — 241,928. They are gathered into families of causes, and the area of each family is exactly its share of the total. The red block, the heart and the blood vessels, takes up more than half the screen on its own.

The recount

A death at 88 and a death at 41 are, here, one dot each.

Count them differently: not people, but the years missing up to 75. The canvas rearranges. The heart falls from 545 to 297 dots. External causes climb from 35 to 113, the liver and the rest of the digestive system from 59 to 110, and the pink that was barely visible — newborns and congenital malformations — jumps from 3 to 33.

The ages

The same thousand dots, spread across the age at which each death happened.

The columns grow to the right: 21.5% of deaths come before 65, the rest after. The colours change along the way. The blue of external causes gathers on the left, in the thin columns of the young; the red of the heart thickens only after 70.

One band at a time

At 15–19, more than half of all deaths come from external causes: the road, the water, one's own hand. At 90–94, eight in ten come from the heart and the vessels. The question “what do Romanians die of” has no single answer, but twenty-one.

What moved

Two counts, the same dead

A picture of causes of death says who dies most often. It does not say how much is lost. Those are two different questions and, in Romania, they produce two different pictures.

The first count is simple: every person recorded dead in 2023 is worth one. That is how you arrive at the 241,928 deaths and at the red block that fills half the screen. It is the count that produces headlines about the “number one killer”.

The second asks how many years are missing. The international convention is to count the years up to 75 and stop there: a death at 41 is worth 34 years lost, a death at 88 is worth zero. Added up that way, the 241,928 deaths become 1,438,372 years of life lost in a single year, and 110,883 deaths carry all of them.

Moving from one unit to the other shifts the weight from the diseases that close long lives to the ones that cut lives short. The red block stays the largest, but it is no longer a majority. Around it rise four things the first canvas had all but hidden.

The biggest jump belongs to those who have only just been born. Newborn conditions, congenital malformations and deaths linked to pregnancy came to 658 deaths in 2023, three dots out of a thousand. Because each of them takes almost 75 years at once, in the second count they occupy 33. One such death weighs as much as twelve ordinary ones.

Accidents and suicides make the same journey with larger numbers: 8,491 deaths across the whole year, but 163,397 years lost. And cirrhosis together with the other chronic liver diseases, 7,850 deaths, reach 100,855 years — more than all cerebrovascular disease combined, which kills four times as many people.

Cancers are the only large family whose place barely changes: 192 dots by deaths, 247 by years. They strike early enough to lose nothing in the recount and often enough to stay second everywhere.

Fifteen rows, two orders

The families above gather very different diseases. Opened up into individual causes, the two counts produce two rankings that agree only at the top.

The bar shows the size relative to the first row. “The rest of circulatory disease” is a residual category of the classification, not a disease; see the limits below.

The same unit, another continent

Where the distance from Europe sits

Measured as years lost per hundred thousand inhabitants, with the age structures brought to a common standard, Romania loses 7,938 and the European Union 4,767. A factor of 1.67. The distance, though, is not spread evenly.

Most is lost where European medicine has had cheap answers for decades. In lung disease, where pneumonia alone accounts for six in ten deaths, Romania loses more than three times as many years as the Union average. In infectious disease, where tuberculosis still kills 705 people a year, almost two and a half times. In liver disease, almost three times. In heart disease, two and a half.

For cancers the gap narrows to 1.3. It is the only large family where Romania sits close to Europe, and not because the treatment is good: for many cancers, people die before reaching the age at which cancers appear. The inverse reading is more useful: if heart, lung and liver disease came down to the European average, what would remain is a picture of death in which oncology becomes the main problem. That is where the countries of western Europe have arrived.

The same distance shows up in something far simpler to check: 21.5% of deaths in Romania come before the age of 65, against 14.9% in the Union. One in five, against one in seven.

How the counting was done

Method and limits

Every figure comes from the Eurostat table hlth_cd_aro, deaths by cause and age for a country's residents, extracted on 7 September 2026. 2023 is the latest year published for Romania. The populations used for the European rates come from demo_pjan, at 1 January 2023. The recorded cause is the underlying cause of death, the disease or event that began the chain, as coded under ICD-10.

Years of life lost follow the usual convention: for every death under 75, the difference between 75 and the midpoint of the age band in which it was recorded is added up, and deaths at 75 and over count zero. The threshold is a convention, not a property of nature; at 70 or at 80 the order of the families shifts a little, but the direction of the move stays the same. The rates in the European section are standardised on the 2013 European Standard Population over ages 0–74, and read as years lost per 100,000 inhabitants.

The thousand dots are divided by the largest-remainder method, so that every canvas holds exactly a thousand. One dot means 242 deaths in the first count and 1,438 years in the second. For a single age band, a dot is a thousandth of that band's deaths, and its meaning changes from band to band.

Three limits that matter

The largest category is a remainder. “The rest of circulatory disease” gathers 45,457 deaths, more than ischaemic heart disease itself. Underneath it sit, for the most part, forms of heart failure and atherosclerosis entered when the disease that began the chain is not known. The red block is reliable as a size; inside it, the borders are softer than they look.

Dementia does not appear at all. For 2023 Romania reported zero deaths under codes F01–F03, dementia, against 176,161 in the European Union. Alzheimer's disease appears separately, with 1,868. The difference is not biological but a matter of how certificates are filled in: here, a person with dementia is usually recorded with the disease that actually killed them. The “other illness” family on the canvas is therefore undercounted, and its comparison with Europe should not be read as an advantage.

One year is not a history. 2023 is the first full year without the pandemic's direct effect on the total number of deaths, yet COVID-19 still appears with 1,888 deaths. The shapes on the canvas change slowly from year to year; the exact figures do not.
