One second without air

01 / the switch

Two routes, one moment

Air travels towards the lungs, while a food bolus travels towards the oesophagus. Swallowing does not press one isolated button. It starts a rapid sequence: breathing is inhibited, the larynx rises, the laryngeal entrance closes and the bolus moves towards the oesophagus.

After the swallow, exhalation is common. This return of outward airflow helps clear residue from the pharynx and explains why the exhale–swallow–exhale pattern appears in most discrete thin-liquid swallows.

The pause has a measure

In a study of 12 healthy young adults, 72 swallows produced a mean deglutition apnea of 0.75 ± 0.14 seconds. Exhalation preceded the pause in 93% of trials and followed it in every trial.

A meta-analysis of 37 articles found the exhale–swallow–exhale pattern in an average of 77.4% of thin-liquid swallows. The percentage is not constant: volume, consistency, presentation and age change the pattern.

Deglutition apnea is a coordinated pause, not a test of endurance. The figures describe groups and experimental tasks, not every swallow.

Watch the switch work

You can observe the mechanism without holding your breath. Sit comfortably, take a small sip of water and notice what happens after it: in many adults, the next movement of air is an exhalation.

  1. Breathe normally and feel the rhythm of your abdomen or chest without changing it.
  2. Swallow a small sip of water without extending the pause or repeating the swallow quickly.
  3. Notice whether air returns on an exhalation. Compare the sensation with the map, not with a diagnosis.

Stop if you develop persistent coughing, pain or a choking sensation. This page does not assess dysphagia, aspiration or lung function.

A map, not a test

The calculator divides pause duration by the length of one breathing cycle, calculated as 60 ÷ breathing rate, then expresses the result as a percentage. It does not estimate aspiration risk and cannot replace medical assessment.

The epiglottis contributes to airway protection, but the mechanism is not a single flap. Sources describe respiratory inhibition, laryngeal movement, laryngeal-vestibule closure and the return of exhalation together.
