FIT kitBlood pressureCholesterolBlood sugarDEXAHepatitis B, CHIV
  1. At 35

    • Cholesterol and 10-year heart and stroke risk, every 5 years
    • Blood sugar test for diabetes, every 3 years (if you are at risk; how often depends on your risk)
  2. At 40

    • Blood pressure, every year
  3. At 50

    • Bowel cancer screening (FIT kit), every year
  4. At 70

    • Bone density scan (DEXA), every 5 years
  5. At 71

    • Stops: Blood sugar test for diabetes
  6. At 75

    • Stops: Bowel cancer screening (FIT kit)

After 74โ€“75 most screening is no longer recommended routinely, because its benefit takes years to arrive. Whether to continue is a decision with your doctor, based on your health and wishes.

What each test is worth

The same question for each: per 1,000 people, how many deaths from the disease does the test prevent? Some numbers come from trials (people invited, or randomly assigned to the test), others from a model that assumes people always attend; each has its own time span, written beside it. For osteoporosis the number is hip fractures, not deaths. So the tests are not ranked on one scale.

Each small square is a person; there are 10,000.

Blood pressure

18โ€“39, every 3 years (or every 5 years if normal); from 40, every year

High blood pressure, which causes no symptoms and raises the risk of heart attack and stroke.

no death count for the test

No trial compares people whose blood pressure was measured with people whose was not. The benefit comes from treatment, which clearly cuts heart attacks and strokes.

What the evidence says

Measured from 18: every 3โ€“5 years up to 40 if normal, then every year (US guidance, USPSTF).

Leumit, routine screening tests by age, USPSTF, Hypertension in adults: screening (2021)

How to get it

Through your health fund (kupat holim), with a referral from your family doctor; routine blood tests are in the health basket.

Leumit, routine screening tests by age, USPSTF, Hypertension in adults: screening (2021)

Hepatitis B and C, one-time test

from 18, once, if never done (again after any new risk)

Viruses that damage the liver silently for decades, up to cirrhosis or cancer. Hepatitis C is now cured with tablets in 8โ€“12 weeks.

no death count for the test

No death count has been measured for the test itself. The benefit comes from treatment: hepatitis C is cured in over 95% of people treated.

What the evidence says

No general screening programme; testing is advised after any possible risk.

How to get it

Through your family doctor at your health fund.

HIV, one-time test

from 18, once, if never done (again after any new risk)

HIV infection, which treated in time means a near-normal lifespan.

no death count for the test

The benefit comes from starting treatment early; no death count has been measured for the test itself.

What the evidence says

No general programme; testing is recommended after any possible risk.

Ministry of Health, recognised HIV testing clinics

How to get it

Free through your health fund (not anonymous), or free and anonymous at the Ministry of Health's sexual health clinics in Tel Aviv and Haifa and at Israel AIDS Task Force centres.

Ministry of Health, recognised HIV testing clinics

Bowel cancer screening (FIT kit)

50โ€“74, every year

Invisible traces of blood in stool, a sign of polyps and bowel cancer. A positive test leads to a colonoscopy, where polyps can be removed there and then.

If a parent, brother, sister or child had bowel cancer, don't wait for the programme: colonoscopy starts at 40, or 10 years before the age your relative was diagnosed, whichever comes first. Tell your doctor.

24โ€“28 per 1,000

bowel cancer deaths avoided per 1,000 people screened from 45 to 75, modelled estimate (US)

modelled estimate

The model assumes a FIT every year (or a colonoscopy every 10 years) from 45 to 75. The Israeli programme, every year from 50 to 74, is close to it.

What the evidence says

A stool test for hidden blood (FIT) every year from 50 to 74; a positive test leads to a colonoscopy.

Ministry of Health, occult blood in stool: the national colorectal screening programme, Leumit, routine screening tests by age

How to get it

Free; get the kit from your health fund (clinic, app, or by post, depending on the fund).

Ministry of Health, occult blood in stool: the national colorectal screening programme, Leumit, routine screening tests by age

Cholesterol and 10-year heart and stroke risk

from 35, every 5 years

Your chance of a heart attack or stroke in the next 10 years, worked out from cholesterol, blood pressure, age and smoking.

no death count for the test

The benefit comes from treatment: in 18 trials of adults with no previous heart attack or stroke, statins meant about 3.5 fewer deaths per 1,000 people treated, over 6 months to 6 years.

What the evidence says

A cholesterol test every 5 years from 35; for older adults the Ministry advises yearly.

Leumit, routine screening tests by age

How to get it

Through your health fund (kupat holim), with a referral from your family doctor; routine blood tests are in the health basket.

Leumit, routine screening tests by age

Blood sugar test for diabetes

35โ€“70, every 3 years (if you are at risk; how often depends on your risk)

Type 2 diabetes and prediabetes, which can go years without signs.

no death count for the test

Screening trials found no fewer deaths over 10 years. The test is worth it if something changes afterwards: diet, activity, treatment.

What the evidence says

A fasting blood sugar test from 35 in people at risk.

Leumit, routine screening tests by age, USPSTF, Prediabetes and type 2 diabetes: screening (2021)

How to get it

Through your health fund (kupat holim), with a referral from your family doctor; routine blood tests are in the health basket.

Bone density scan (DEXA)

Women: from 65, every 5 years; Men: from 70, every 5 years

Osteoporosis: bones that break in an ordinary fall.

5 per 1,000

hip fractures prevented per 1,000 older women, over 4โ€“5 years (three trials)

measured in trials

What the evidence says

A bone density scan (DXA) every 5 years: women from 65, men from 70.

Leumit, routine screening tests by age, Ministry of Health, periodic check-ups for older adults

How to get it

On referral from your family doctor; a co-payment may apply.

Leumit, routine screening tests by age, Ministry of Health, periodic check-ups for older adults

Cervical screening

25โ€“54, every 3 years (an HPV test); 55โ€“64, every 5 years (an HPV test)

High-risk types of HPV, which over years can turn cells of the cervix into cancer. A positive test leads to a check of the cells and, if needed, a colposcopy, where changes are found before they become cancer.

8 per 1,000

cervical cancer deaths avoided per 1,000 women with an HPV test every 5 years from 30 to 65, compared with none: from 8.34 to 0.29 (modelled estimate, US)

modelled estimate

What the evidence says

An HPV test, which has replaced the Pap smear: every 3 years from 25 to 54, every 5 years from 55 to 64. After 65, one more test if you haven't had one in the past 10 years.

Maccabi, the HPV test, Leumit, the HPV cervical test

How to get it

Free in the health basket; book it through your health fund's app or clinic.

Maccabi, the HPV test, Leumit, the HPV cervical test

Breast screening (mammogram)

50โ€“74, every 2 years

Breast cancer while it is small, before it can be felt.

4.3 per 1,000

breast cancer deaths prevented per 1,000 women invited from 50, over 20 years

estimated from 11 trials

12.9 cancers found that would never have caused harm (overdiagnosis)

What the evidence says

A mammogram every 2 years from 50 to 74. Women with a BRCA mutation or a strong family history start earlier.

Leumit, routine screening tests by age

How to get it

Free in the health basket, with no co-payment, every 2 years from 50 to 74; your health fund invites you.

Leumit, routine screening tests by age

Lung scan (low-dose CT) for smokers

60โ€“74, every 2 years (20+ pack-years; smoking now or quit within 15 years)

Lung cancer at a stage when it can still be operated on.

3 per 1,000

lung cancer deaths prevented per 1,000 heavy smokers scanned yearly, over about 7 years, compared with chest X-ray (NLST)

measured in a trial

Nearly a quarter of scans come back positive at first; almost all turn out to be false alarms.

What the evidence says

A low-dose CT every 2 years from 60 to 74, for people with at least 20 pack-years who smoke or quit within the last 15 years; added to the health basket in 2026.

Ministry of Health, circular on the 2026 health basket (lung LDCT screening), Maccabi, low-dose CT for lung screening: eligibility

How to get it

On referral from your family doctor, at approved imaging centres; a small co-payment may apply.

Ministry of Health, circular on the 2026 health basket (lung LDCT screening), Maccabi, low-dose CT for lung screening: eligibility

Abdominal aortic aneurysm (AAA) scan

65โ€“75, once, if never done

A swelling of the main artery in the abdomen that causes no pain and no signs until it bursts.

4.6 per 1,000

aneurysm deaths prevented per 1,000 men invited to one scan, over 13 years (MASS trial)

measured in a trial

In December 1948 the surgeon Rudolph Nissen opened Albert Einstein's abdomen in Brooklyn and found a large aneurysm on the aorta. There was no way to replace it, so he wrapped it in cellophane to stiffen it. Einstein lived six more years. In April 1955 the aneurysm burst; he refused surgery and died on 18 April. Today the same swelling shows up on a scan that takes a few minutes.

What the evidence says

One ultrasound of the aorta for men 65 to 75 who smoke or used to.

Leumit, routine screening tests by age

How to get it

Through your family doctor at your health fund.

Leumit, routine screening tests by age

PSA test for prostate cancer

Prostate cancer, including many small tumours that would never have caused harm.

If your father or brother had prostate cancer, or you are of African descent, the PSA conversation starts at 45; with a BRCA2 mutation, at 40 (European urology guideline, EAU).

2.2 per 1,000

prostate cancer deaths prevented per 1,000 men aged 55โ€“69 invited to PSA testing, over 23 years (ERSPC)

measured in a trial

More than 45% of men tested at every round had at least one false positive over 10 years, and 20โ€“50% of the cancers found would never have caused harm.

What the evidence says

No national programme; the Israel Cancer Association advises men 55 to 69 to discuss a PSA test with their doctor.

Israel Cancer Association, prostate cancer early detection

How to get it

Talk it through with your family doctor before any test.

Israel Cancer Association, prostate cancer early detection

Skin cancer check

Melanoma and other skin cancers, through an examination of all your skin, scalp included.

no death count for the test

No trial has shown fewer melanoma deaths from checking the skin of people without symptoms; the checks do find many spots that lead to biopsies.

What the evidence says

No screening programme. Check your own skin and see your doctor about a mole that changes.

USPSTF, Skin cancer: screening (2023)

How to get it

See your family doctor about any mole that grows, changes or bleeds.

USPSTF, Skin cancer: screening (2023)

What isn't worth the money

Private clinics sell 'complete' check-ups. The tests below often appear in them and have no evidence of saving lives in people without symptoms and at average risk; some cause harm. Symptoms need a doctor, not a screening list.

Common questions

Which screening does the health basket cover in Israel?

In the basket: an HPV test every 3 years from 25 to 54 and every 5 from 55 to 64; a mammogram every 2 years from 50 to 74; a stool test every year from 50 to 74; a low-dose lung CT every 2 years from 60 to 74 for heavy smokers; cholesterol from 35. Everyone insured with a health fund is covered.

Which tests should you have at 40?

Women: Cervical screening, Blood pressure, Cholesterol and 10-year heart and stroke risk, Blood sugar test for diabetes, Hepatitis B and C, one-time test, HIV, one-time test. Men: Blood pressure, Cholesterol and 10-year heart and stroke risk, Blood sugar test for diabetes, Hepatitis B and C, one-time test, HIV, one-time test.

Which screening should you have from 50?

Women: Cervical screening, Breast screening (mammogram), Bowel cancer screening (FIT kit), Blood pressure, Cholesterol and 10-year heart and stroke risk, Blood sugar test for diabetes, Hepatitis B and C, one-time test, HIV, one-time test. Men: Bowel cancer screening (FIT kit), Blood pressure, Cholesterol and 10-year heart and stroke risk, Blood sugar test for diabetes, Hepatitis B and C, one-time test, HIV, one-time test. From 60 to 74, with at least 20 pack-years, a low-dose lung CT every 2 years is in the health basket. Ask your family doctor for a referral.

Which screening should you have from 65?

Women: Breast screening (mammogram), Bowel cancer screening (FIT kit), Blood pressure, Cholesterol and 10-year heart and stroke risk, Bone density scan (DEXA), Blood sugar test for diabetes, Hepatitis B and C, one-time test, HIV, one-time test.

Is a private full check-up worth it?

Usually not. A Cochrane review of 17 trials found general health checks made no difference to deaths. Tumour markers and whole-body scans in people without symptoms raise many false alarms. What matters is the screening that fits your age.

How the list was made

The evidence column follows the Israeli Ministry of Health and the health basket; for the size of each benefit, the randomised trials and the US Preventive Services Task Force evidence reviews. Where Israel has no programme, the page says so.

Lives saved are absolute: deaths avoided per 1,000 people, not '42% fewer'. Some come from trials (people invited, followed for a fixed number of years), others from models that assume you take every test for life. They are labelled separately and not ranked.

This page does not replace your doctor. With symptoms, a family history of cancer or a long-term condition, your list is different and is made with them.

Why the ages change

Screening ages move when the evidence moves, and lately they have moved down because some diseases now appear earlier: in 2021 the US lowered bowel screening from 50 to 45 as bowel cancer rose in younger adults, and diabetes testing from 40 to 35; Spain and Austria widened their programmes in 2025โ€“2026. Every rule here was checked in September 2026. Screening finds disease early; it does not prevent it. The other half stays the same: not smoking, a healthy weight, moving more and drinking less.
